Secretariat forms

For Students

1. General Application

Submitted to the Secretariat by any interested party to formally state their request.

2. Application for Issuance of Certificates

Submitted to the Secretariat by any interested party, after selecting their status and the specific certificate they wish to receive.

3. Application for Suspension of Studies

Submitted to the Secretariat by any interested student at the beginning of the academic semester, stating the duration and the reason for requesting the suspension of their studies.

4. Application for De-registration from the Department

Submitted to the Secretariat in the event that a student wishes to be removed/de-registered from the Department.

5. Application for Bachelor's Thesis Allocation

Submitted to the Secretariat by students in their 7th or higher semester of studies who wish to initiate the preparation of their thesis.

6. Application for Extension of Bachelor's Thesis Completion

Submitted to the Secretariat by students who request a time extension of one (1) academic year to complete their thesis due to serious reasons.

7. Application for Grade Improvement / Re-examination

Submitted to the Secretariat by students who wish to be re-examined in courses they have already successfully passed in order to improve their grade, in accordance with the Department's Internal Regulations.

8. Application for Graduation Clearance

Submitted to the Secretariat by any student who has successfully passed all the courses required to obtain their degree, in order to initiate the official graduation and degree-awarding process.

9. Application for Lateral Admission

Submitted to the Secretariat within the announced deadlines by graduates of other University Faculties who wish to participate in the Department's ranking/lateral admission exams, accompanied by the required supporting documents.

10. Application for Course Recognition

Submitted to the Secretariat by students who enroll in the Department via transfer or lateral admission and wish to recognize course grades they have successfully passed at their institution of origin.

11. Graduate Solemn Affirmation Form and Declaration

Completed by prospective graduates on the day of their graduation ceremony and submitted to the Secretariat. It aims to record/update students' contact details, return student documents, and declare that there are no outstanding obligations to the library and student residences.

12. Graduate Oath

A form provided to students prior to their graduation ceremony, containing the oath they are to recite.

13. Statutory Declaration (Blank)

A blank Statutory Declaration completed with free text by students for any matter concerning them for which no other specific form is available.

14. Statutory Declaration for Loss of Student ID (Pass)

A Statutory Declaration submitted to the Secretariat by students in case of loss of their student identity card (pass), requesting the cancellation of the existing card and the issuance of a new one.

15. Statutory Declaration on the Processing of Personal Data by the Secretariat

A statutory declaration provided to students during their identification process (finalization of enrollment) regarding their notification about the retention and processing of personal data by the Secretariat. It is signed and filed by the Secretariat in each student's personal record.

16. Special Educational Needs Declaration Form

Submitted to the Secretariat by students with special educational needs, either upon enrollment in the Department or at any point during their studies.

Student Forms for Faculty Members

1. Official Memo for Student Grade Correction

An official memorandum signed by the instructor and the Head of the Department, submitted to the Secretariat in the event of a student's grade correction on the official grade roster.

2. Official Memo for Submitting an Omitted Student Grade

An official memorandum signed by the instructor and the Head of the Department, submitted to the Secretariat to report a student's grade that had not been originally entered into the official grade roster.

3. Exam Attendance Certificate

Completed by the Instructor/Examiner after the course examination and given to the student upon their verbal request.

4. Bachelor's Thesis Examination Minutes

Submitted to the Secretariat, signed by the Supervising Professor and the co-examiner, in order to officially register the student's thesis grade.
